Our services provide each individual with a customized health program tailored to meet his or her physical, emotional, and spiritual needs.
St. Johnsville Rehabilitation & Nursing Center began 25 years ago as a 120-bed nursing care facility. Today, the facility is the community's largest employer with over 170 employees offering services that range from outpatient rehabilitation to short and long-term nursing care.
According to Dave Wallace, Administrator at the St. Johnsville Rehabilitation & Nursing Center, the level of patient care needed has changed over the years. Today, hospitals are focused on decreasing lengths of stays and discharging patients earlier with the idea of continuing care through sub-acute, skilled, and outpatient services. This transition in care has prompted the Center's expansion of services provided by on-site specialist physicians, not just primary care physicians ̶ as is often the case in other facilities.
As the only service that provides outpatient rehabilitation in the community, The Center offers an extensive array of clinical and out-patient services including: IV therapy, respiratory therapy, enteral therapy, hydrotherapy, orthopedic treatment for acute chronic neck and low back pain, massage therapy, hand therapy, cumulative trauma disorder treatment, home skills retraining, sports medicine rehabilitation and certified personal trainer services.
Short and long-term services are also extensive and include: physical therapy, occupational therapy, speech and language pathology, podiatry services, dental services, lab & x-ray services, wound care, pain management, post-surgical care, respiratory therapy, palliative and hospice care.
The Center's reputation for providing outstanding care was recognized in 2011 by the American Health Care Association and National Center for Assisted Living with their Bronze-Commitment to Quality National Quality Award. This distinction highlights facilities nationwide for their intention to pursue a rigorous quality improvement.
While the vast array of quality services offered by the facility is impressive, what sets them apart from other skilled nursing centers is their resident-centered care. The Center's staff, family, and residents work as a team to create a personal, more home-like environment for residents.
The Center is also very tech savvy when it comes to improving efficiencies and reducing documentation errors. They have an electronic medical records system and electronic hands-free nursing voice assistant that assist nursing staff facilitate an even more efficient work flow. "We're a small country nursing home that you wouldn't typically find working with leading-edge technology, "stated Wallace. "However, this technology allows us to provide better care with a proactive approach to process improvement and preventing errors," added Wallace. "When an issue is alerted by one of the systems that is used to promote quality, we can immediately address the issue then tie in training for the nursing staff to correct it, thus improving the quality of our care," concluded Wallace.
